Supplemental Accident Coverage for NFHS Professional Association Members

As a currently registered member of an NFHS Professional Association, you receive many benefits of membership, one of which is a package of insurance coverages.

For those associations who wish to purchase a higher limit of Accident Insurance, along with a lower deductible, Bollinger has developed this Supplemental Accident Insurance Program for NFHS member associations.

This program is only available to currently registered members of the following NFHS Professional Associations:


 

Supplemental Accident Policy Benefits

Accident Medical/Dental Limit:	$100,000
Deductible Per Claim:	$100
Benefit Period:	52 weeks
Accidental Death Benefit:	$10,000
Dismemberment Benefit:	$10,000

Official’s Game Fee Reimbursement

If you are injured while performing your duties as an umpire or a sports official and cannot umpire or officiate, the plan will pay:

• Up to $200 per week for lost officiating or umpiring game fees
• Up to $5,200 maximum reimbursement per claim
• Benefit Period: 26 weeks
• 7-day Waiting Period

Annual Premium

Accident Insurance Only:	$8.00 per member
Accident Insurance plus Game Fee Coverage:	$10.00 per member
Minimum Premium:	$500 per NFHS Association per year

To Apply for Coverage

Simply print out the brochure. Complete the application and include a roster of your members (including name and address for each one). Mail the application with your check to Bollinger, PO Box 390, Short Hills, NJ 07078. Coverage will be bound on the day after postmark on your application and check.
